Images appearing several times in pdf document in different orientation are not processed correctly by pcl/afp drivers. Instead, the orientation used at the first time the image appeared in pdf is applied.
The reason is that pdf interpreter keeps the orientation of the first occurance of the image. Then if the same image appears in different orientation CTM matrix is used to specify this orientation. This matrix can be efficiently used by pdf.ps drivers which are capable of placing images in arbitrary orientation. However CTM matrix is ignored. The solution is to use -Orientation- field rather than CTM matrix in pdf interpreter for those simple rotations.
As well, there were some issues with pcl driver related to -Orientation- processing. Corrected those as well.
